Improving Care at the End of Life
Overview
NHS Rotherham is holding an event in August 2012 to develop end of life services for the future. In order to understand the key issues experienced by the people of Rotherham, we would value your input.
All responses will remain anonymous, so please be honest in your answers.

Why We Are Consulting
Your experiences of caring for someone at the end of their life (if this has happened in the last 2 years), or your current experiences as a patient, will inform future commissioning decisions, and will form a key part of the information presented at the August event.
What Happens Next
All responses will be fed into the planned event on end of life care planned for August 2012.
Feedback from both this process and the event will appear on NHS Rotherham's website, and will be displayed on posters in Rotherham Hospice.
