Getting to the Heart of the Matter

[Results expected 9 March 2012 & feedback expected 12 March 2012]

In July 2011, LINk Rotherham and NORCOM organised regional work to look at services and support for people discharged from Cardiac Services in Rotherham.

It was felt that when a patient is diagnosed with Heart Failure then they are assigned a Specialist Nurse who is able to offer support, advice and information when required. However, if somebody has a heart attack, heart operation or other heart condition they are often treated and then discharged with no additional support other than their GP.

A consultation day was held to look at the issues raised and consider ways of future working.

	VASCULAR SERVICES REVIEW

The Yorkshire and Humber Specialised Commissioning Group (SCG) has conducted a review of vascular services on behalf of the 14 primary care trusts in Yorkshire and the Humber region and is proposing to reorganise the way that these specialised services are provided in order to improve the care, quality and safety of our patients.

	What are vascular services?

Vascular services consist of planned treatment for conditions where there is not enough blood reaching an organ or parts of the body such as the arms, legs or head, caused by a partial or total blockage. Vascular services also include planned treatment for aneurysms, a fluid-filled bulge in an artery that can weaken it and treatment for other types of abnormal blood vessels. In addition, vascular specialists are needed to support other medical treatments such as kidney dialysis or chemotherapy access. As well as planned treatment, vascular services also include emergency treatment, including life threatening emergencies, such as when a large artery bursts; where there is a critical lack of blood to a limb, when the lack of a blood supply can be limb threatening; or injuries from road traffic accidents.

A MAJOR public consultation is underway to find out what people living in the Doncaster area think about proposals to reorganise health services at two of the borough’s hospitals and provide more care in the community.

Over the next three months NHS Doncaster, the borough’s primary care trust, is asking for views on a number of proposals, including:

closing Ash, Elm and Rowan wards at Tickhill Road Hospital, Balby and relocating the service they provide into a new multi-million-pound rehabilitation centre which would be built on the Montagu Hospital site at Mexborough to serve all the Doncaster area.

expanding local health and community care services

carrying out more day surgery at Montagu Hospital to reduce the length of time that patients have to wait for an operation.

stopping admitting to Montagu Hospital those very ill patients who need a ‘blue light’ ambulance and taking them straight to Doncaster Royal Infirmary or another district general hospital - such as Barnsley or Rotherham – instead.

doubling the number and type of endoscopy investigations that Montagu Hospital carries out - up from four half day sessions each week to eight.

[Results expected 28 January 2011 & feedback expected 25 February 2011]

	MORBID OBESITY SURGICAL SERVICES

What are morbid obesity surgical services?

Body Mass Index (BMI) is a calculation used to work out how overweight people are by using their weight and height measurements to determine a score. People considered a healthy weight have a BMI between 20kg/m2 and 25kg/m2. A person is usually considered morbidly obese when they have a BMI of over 40 kg/m2. A morbidly obese person is considered to have a body weight high enough to pose a severe risk to their health, which could mean that they have a reduced life expectancy and/or increased health problems.

Morbid obesity surgery, often referred to as bariatric surgery (which could include gastric band or gastric bypass surgery), is a recommended treatment option for people who are morbidly obese

Review of morbid obesity surgical services in the Yorkshire and Humber region

The National Institute for Health and Clinical Excellence (NICE) is an organisation that advises the NHS when a clinical treatment (or set of clinical procedures) is considered highly clinically effective, cost effective and safe.

NICE has issued some guidelines that PCTs may consider when commissioning morbid obesity surgery. These take into account the clinical effectiveness of treatment, money saved due to the reduction in weight after surgery and any costs of aftercare.

Primary Care Trusts (PCTs) are responsible for funding NHS treatment locally. The PCTs in this region are currently reviewing the criteria for funding surgery for individuals who are morbidly obese.

Yorkshire Ambulance Service is looking to apply for foundation trust status in 2012.

NHS foundation trusts are membership organisations that are free from central government control. This means that the new organisation will have a lot more freedom to shape the way that it provides and develops services for local residents.

 Duty of Candour National Consultation

A consultation has been launched on implementing a ‘Duty of Candour’, a contractual requirement on NHS providers to be open with patients when things go wrong with their healthcare.

The contractual Duty of Candour, which forms part of the Government’s plans to modernise the NHS by making it more accountable and transparent, will be an enforceable duty on providers.
